It’s very quite common for most of the people to expierence earpain in middle of night,disturbing the sleep.Earpain/earache can be due to problem with ear or structures close to the ear.

The pain may be dull,sharp or burning and can occur in one or both ears.It may constant and come and go.Earpain sometimes associated with fever.itching in ears,decreased hearing,ear discharge,rarely vertigo,nausea,vomiting etc.

Usually earpain in children follows cold attack,which causes Eustachian tube block.Eustachian tube runs middle part of each ear to back into nose.Eustachian tube block happens due to viral cold give rise fluid to build up in middle ear and cause ear pain in children.

Ear pain in adults is less likely to be from middle ear infections .Common causes of earpain in adults otitis externa i.e ear canal skin infections and TMJ dysfunction.

Swimmers ear is an infection of ear canal skin and results from swimming in contaminated water or result of earbud/cotton swab use.In people with diabetes,otitis externa can spread beyond ear skin into underlying bones,can cause life threatening infections.Therfore,prompt treatment with antibiotic ear drops as well as cleaning of ear with special equipment like suction and microscope is needed.

Another common cause for earpain is due to referred pain from the jaw joint.This is usually due to grinding of teeth during sleep.

Deafness

When you feel you are difficulty in understanding in noisy environment or if you are missing some words while conversation or if you need to rise TVvolume ,then it is time to consult ENT doctor,get checked for hearing.Reasons could be as simple as -wax accumulation in ear,or sometimes it is complex like brain tumour

ENT specialist suggests you to go for audiometry test and based on this doctor identify nature of hearing loss whether it is conductive or sensorineural or mixed.One may need further tests like CT scan,MRI for arrive correct diagnosis apart from audiometry test.
